Of the 198 collectible documents, 93 are hidden in Chapter locations and 38 in side missions, and 48 documents are scattered around The City. All of these can be found at any time once their initial requirements are met, if any. If you miss one, don’t worry because you can come back for it later.

The Newspapers, however, are a little tricky. All of the Newspapers in Stonemarket and 3 (2?) in the South Quarter are available for the rest of the game once they initially appear, but 7 (8?) of the 19 Newspapers can only be found in The City at very specific times. For example, “CHARLATAN REMEDIES” can only be found between Chapters 3 and 4 at the Siren’s Rest, and if you come looking before or after that window you will only find a pixely shadow where it either will be or used to be.

Many forum threads you’ll find on the internet propose or claim that some of the documents are missing from the game entirely but I have personally found each and every one. I’ve double- and triple-checked nearly all of them and it is indeed possible to collect 100% of the Documents, as long as you are mindful of a few Newspapers’ limited appearances.
